10 Options in Treating Mesothelioma

10 Options in Treating Mesothelioma

As with any cancer, treatment for Mesothelioma depends on the location of the cancer, the progression of the disease, the age as well as state of health of the patient.

It is the oncologist who specializes in the disease who will decide on the line of treatment. The disease is difficult to treat and in most cases the prognosis is poor. Treatment options include:

More Information on Mesothelioma

1.    If the diagnosis is done in the early stages surgery may be recommended to remove all cancerous tissues. This means thoracoscopy, VATS or video assisted thoracic surgery, mediastinoscopy (used for staging), or laproscopy.  Often, doctors will advise palliative procedures like chest tube drainage and pleurodesis, thoracoscopy and pleurodesis, pleuroperitonial shunt, or pleurectomy, which treat the symptoms of mesothelioma rather than the disease.

2.    Radiation is prescribed aggressively for mesothelioma patients and is often given in combination with surgery or in order to control symptoms, palliatively. Research on using radiation therapy using implants or UV light therapy is in progress as traditional radiation therapy damages surrounding healthy tissue.

3.    Chemotherapy is an option and around 12-20% of patients respond to the drugs. Anti-cancer drugs destroy cancer cells and prevent their spread. In mesothelioma, chemotherapy is not considered to be curative. The aim is to prevent spread of the disease; shrink the tumor prior to surgery, known as neoadjuvant therapy; to annihilate any remains of the tumor in the body post surgery; and to relieve pain and other discomfort, palliative chemotherapy. Experts recommend prescribing premetrexed along with cisplastin. These drugs have shown positive results and this is now standard care for Mesothelioma not treatable by surgery.

4.    Biological therapy using interleukin 2.

5.    Immunotherapy where the body’s own immune system is stimulated to fight the disease. Biological response modifiers, BRMs, are used in treatment. These enhance the functions of the immune system; regulate metabolic reactions that promote growth of cancers; alter cancer cell division; block or reverse processes that lead to formation of cancers; and prevent spread of cancer.

6.    Gene therapy, where attempts are made to correct the gene sequence that causes the cancer. This is of two kinds, replacement gene therapy and knockout gene therapy.

7.    Administration of lovastatin, a cholesterol lowering drug.

8.    Photodynamic treatment where a photosensitive medication is activated using a laser. This is done during the surgical removal of cancerous tissues.

9.    Draining of fluid in the chest or abdominal cavity followed by introduction of medications into the cavity to prevent fluid accumulation is done for patients who have uncontrolled fluid accumulation and intense discomfort stemming from it.

10.    Combination therapy or Trimodal therapy which means surgery accompanied by radiation and chemotherapy.

Often, complementary medicines are also used. Termed to be holistic this kind of treatment focuses on a patient’s physical, mental, emotional, and spiritual well being.

In the case of Mesothelioma which is untreatable one can opt for clinical trials of new treatments that are in progress in several research laboratories and centers. The best advice on the line of treatment would be that recommended by the oncologist or physician.

Treatment Options for Pericardial Mesothelioma

Treatment Options for Pericardial Mesothelioma

Of all the cancers caused by asbestos dust (mesothelioma), pericardial mesothelioma is the most rare, accounting for a fraction of all cases (5%). It is a cancer that grows on the pericardium (the lining of the heart). This is a delicate ‘film’, and any infection on the pericardium can have deadly consequences.

Once an individual has been diagnosed with pericardial mesothelioma they are offered three different treatment options. These are radiation, surgery and chemotherapy. Pericardial mesothelioma is extremely difficult to treat (due to the intricacies involved in removing the tumours from the pericardium), and a patient’s chances of survival are slim. All three methods have a poor success rate mainly due to the fact that pericardial mesothelioma is detected in the mature stages of development. (The reason for this is because in the early stages of the cancer the symptoms resemble those of diseases such as pneumonia). The prognosis for a patient undergoing treatment at this point (mature stage) is usually less than a year.

Before treatment, the patient requires a doctor’s evaluation in order to determine which of the three treatments are suitable for him/her. Details such as age, weight, medical history, and general well-being are considered prior to the commencement of treatment. So, how do you treat pericardial mesothelioma ?

Radiation – As with all cancer treatments that use this method, large doses of radiation are used to kill the cancerous cells on the pericardium. The downside to radiation treatment is that not only are you killing off the cancerous cells, but you are also damaging the vital organs in close proximity to the heart, namely the lungs. The heart also gets damaged (due to the large doses of radiation).

Surgery – We can break this down into two parts; aggressive surgery and palliative procedures. Aggressive surgery involves the removal of the cancerous cell. Since these are very close to the heart and lungs, this makes it an extremely precarious procedure and one that requires an extremely skilled surgeon.

Palliative procedures are used in the latest of stages of pericardial mesothelioma and serve the sole purpose of reducing the symptoms. Usually at this stage the cancer is practically incurable.

Chemotherapy – This is the most widely-used of cancer therapies which involves the use of drugs to kill the cancerous cells. Unfortunately, the majority of drugs used have a low success rate with pericardial mesothelioma. Research is being conducted to come up with a cocktail of drugs that have a more favourable outcome.

There does exist another form of treatment which involves using all three aforementioned treatment options. It is known as Dual Therapy. Surgery is used to remove the cancerous cells, then chemotherapy and radiation are used to kill the remaining cells. The success rate for this method is higher than just using radiation, surgery or chemotherapy alone, but can still be dangerous.

Treatment Options for Pleural Mesothelioma

Treatment Options for Pleural Mesothelioma

Mesothelioma is a deadly cancer that is directly attributed to asbestos exposure. There are three forms of mesothelioma. These forms are pleural mesothelioma, peritoneal mesothelioma and pericardial mesothelioma. This devastating cancer attacks the mesothelium, or the protective lining of the lungs, abdominal cavity, and heart respectively.

Of the three forms of mesothelioma, pleural mesothelioma is the most common. When asbestos is manufactured, mined or disturbed asbestos fibers are released into the air. Those working with or around asbestos inhale and ingest these fine particles. Over an extended period of time, mesothelioma can take 20 to 60 years to manifest, these asbestos particles can cause normal pleura in the lining of the chest to become abnormal, causing pleural mesothelioma.

Some of the most common symptoms of pleural mesothelioma are persistent cough, shortness of breath, bloody sputum, weight loss, fever, swelling of the face and neck, raspy voice and trouble swallowing. Due to the symptoms of pleural mesothelioma mimicking symptoms of other conditions it may go undiagnosed when the need to start mesothelioma treatment immediately is vital.

After a definitive diagnosis of pleural mesothelioma has been established there are several mesothelioma treatment options that can be undertaken. The type of mesothelioma treatment will depend on several key components. These factors are the extent of the disease, the patient’s history and age, and the location of the tumor.

The most common forms of mesothelioma treatment, for those with pleural mesothelioma, are a pneumonectomy, radiation, chemotherapy, and palliative therapies. One form of mesothelioma treatment is a pneumonectomy, or the removal of the cancerous lung. Depending on the spread of the disease, parts of diaphragm may be removed as well. A pleurectomy may also be used to treat pleural mesothelioma by removing parts of the chest and surrounding tissue. The ability for these procedures to be performed are dependent on the patient’s overall health, spread of the disease, and the amount of asbestos exposure.

Radiation treatment is another form of mesothelioma treatment for those suffering from pleural mesothelioma. Radiation therapy works by placing a radioactive source in the area of disease. The use of radiation has the benefit of destroying cancerous tissue while not exposing healthy cells. This form of mesothelioma treatment can be performed in conjunction with surgery or alone if the individual is to ill for surgery.

Chemotherapy is another common form of mesothelioma treatment for those with pleural mesothelioma. This high toxicity drug is usually administered by injection or in pill form. Some side affects from chemotherapy include nausea, vomiting, hair loss, fatigue and weight loss. This form of mesothelioma treatment is not targeted to a particular area like radiation therapy. Therefore, the drugs used have to make their way through the body to the cancerous area or areas. Unfortunately, this has the disadvantage of exposing healthy tissue to the toxicity of the chemotherapy drugs.

Palliative therapies are also used as a form of mesothelioma treatment for those with pleural mesothelioma. This involves removing excess fluid by needle and suction from the affected areas. Drugs are also used in order to keep fluid accumulation under control.

As additional research is performed on pleural mesothelioma more treatment options may become available. Keeping informed of new mesothelioma treatments is paramount if diagnosed with this devastating disease.

Chemotherapy generally means the use of drugs to treat diseases and ailments, but the term is used more commonly to describe the use of anticancer drugs. These drugs treat cancer by killing the cancerous cells.

Anti cancer drugs work by specifically targeting certain processes in cancerous cells to stop cell division and multiplication ultimately leading to the death of the cancerous cells. These drugs are however not highly specific in their mode of action and in addition to killing cancerous cells, anti cancer drugs also kill normal healthy cells such as those involved in hair growth.

There are several options for chemotherapy of mesothelioma cancer. Most of these drugs have side effects like nausea, vomiting, loss of hair, loss of appetite and decreased immunity which makes them more susceptible to infections. Any symptoms experienced while undergoing treatment with chemotherapeutic drugs should be reported to the doctor.

Many patients will consider chemotherapy as a treatment option. Understanding the various chemotherapy options and available treatments often helps patients and their loved ones make the best decision for their personal situation. Evaluating Chemotherapy Options Deciding the best chemotherapy option for treating specific mesothelioma cases depends on a number of factors, including the type of mesothelioma, the stage of the disease, and other treatment being used by the victim. Factors such as overall physical health and age are also taken into consideration when planning a chemotherapy treatment plan.

Currently more than 100 chemotherapy drugs are on the market but only a handful have proven to be appropriate for fighting mesothelioma. In most cases, doctors treating mesothelioma patients recommend combination therapy – the tandem use of two different chemotherapy drugs. These combination’s have been proven to be the most successful in treating the cancer.

All treatment options will be thoroughly discussed with the patient’s oncologist{specialist cancer doctor} before a decision is made. Patients should provide a full disclosure of their medical history, including details of any prescriptions and over-the-counter medications being taken to ensure treatment is administered with the most appropriate chemotherapeutic agent.

Currently, the most common chemotherapy drugs for mesothelioma include Alimta (the only FDA-approved drug specifically designed for the treatment of mesothelioma), Cisplatin, Carboplatin, Onconase, Gemcitabine, and Navelbine. The medications are usually administered conventionally (by IV or pill form), or may be used in a newer treatment method called heated chemotherapy.

Can Chemotherapy Cure Mesothelioma?

No it can not cure the cancer, currently there is no proven cure for mesothelioma. However, chemotherapy can help control symptoms and shrink tumors and is largely used as a palliative measure to help improve the patient’s quality of life. The development of combination treatments has proved successful in achieving longer survival rates and researchers continue to look for new ways to fight this terrible disease.
